Look for breathing/pulse (carotid in adult/child; brachial in infant)
  Get AED or send someone, before beginning CPR Witnessed collapse - follow Adult steps
Unwitnessed collapse - give 2 minutes of CPR before getting AED

Rescue breaths every 5-6 s
Check pulse every 2 min
Give naloxone if clinically appropriate

2 hands on lower sternum
At least 1/3 AP diameter - 2 in (5 cm)
2 hands on lower sternum
(1 hand if very small child)

At least 1/3 AP diameter - 1.5 in (4 cm)
2-rescuer CPR - 2 thumb-encircling hands just below nipple line
1-rescuer CPR - 2 fingers in center of chest just below nipple line

Adult BLS algorithm

ACLS

Defibrillation (not synchronized)

  • Biphasic - 120-200 J, or max available
  • Monophasic - 360 J

Synchronized cardioversion

  • Start at biphasic 100 J, then can proceed to 150 J, 200 J if does not convert

Meds

  • Asystole - start epinephrine 1 mg q 3-5 min IV/IO
  • Shockable rhythm
    • After 2nd shock - start epinephrine 1 mg q 3-5 min IV/IO
    • After 3rd shock - start amiodarone 300 mg bolus IV/IO
      • Second dose 150 mg after trying epinephrine again
      • Amiodarone only gets two doses, then use only epinephrine
