Carotid endarterectomy

  • External carotid will be medial to ICA
  • First branch of external carotid is superior thyroid artery (or ascending pharyngeal)
  • Hypoglossal nerve crosses at/above? bifurcation

Surgical management

  • Indications for surgical management
    • Asymptomatic - ICA stenosis 60-99%
      • Should have life expectancy > 5 years, low surgical risk
    • Symptomatic carotid disease = focal neurologic symptoms in carotid artery distribution, including TIA, amaurosis fugax, CVA
  • ASA 81 daily for all patients
  • CEA - patch vs eversion
