- Conus ascends with development
- Normal conus terminates at L1-L2 (adult/by 2 mo of birth)
- L3 at 30 wks
- Conus is 'low-lying' if tip is below lower border of L2 vertebral body at birth (lower if premature)
- Stretch-induced dysfunction/injury of caudal spinal cord, caused by attachments of the filum terminale
- Secondary to myelomeningocele, lipomeningocele, caudal regression/sacral agenesis, etc
- Bladder dysfunction varies: both lower motor neuron (low contractility) and suprasacral (detrusor overactivity, DSD)
- Atonic bladder, poor compliance, DESD, ...
- Urodynamics before and after surgery
- Progressive motor and sensory dysfunction, gait abnormalities/stumbling
- Bladder dysfunction
- Back pain
- Scoliosis, foot/equinovarus deformities
- Repair before ***7 years of age?
- Generally around 4-5 mo
